Health Homes provide Care Management services to ensure all those involved in an individual’s care are working together and sharing the necessary information in supporting a person’s recovery. A Health Home Care Manager is expected to help coordinate not just medical, mental health and substance abuse services, but the social service needs of the individual as well.

The Health Home assists you to:

- Advocate on your behalf so you receive the care you deserve
- Navigate the complex healthcare system
- Link to resources for finding affordable housing, food, clothing, childcare and transportation
- Learn more about meaningful activities to improve your health and keep you healthy
- Identify your goals and ensure everyone involved in your care understands your goals and the care plan you created, to best help you reach those goals
